Sorry - I know I've asked you some of these questions before, but I seem to have lost my data (long story but I've had a few problems with my laptop & have had to restore it back & have lost some data along the way - nothing for anyone to worry about).

Please could you complete as much of the following as you feel happy with & email it back to me. This data is purely for my own records & no information is ever passed onto anyone or any 3rd party. Once received, this data will not be stored on my web server - only on my laptop & backup disc, so should be unavailable to hackers.
